TU Dublin Accident Report Form
- All Accidents and injuries must be reported within 24-hours to the Safety, Health & Welfare Office using the online accident report form.
- They must also be reported to the relevant Line Manager/Supervisor/Head of School/Function.
- The accident report form is completed by the injured person or the person to whom the accident was reported.
- Visitors, Contractors/Service Providers or any third party are required to immediately inform their host/contact person if they are involved in an accident.
- If a driver is involved in a road traffic accident whilst on University business, this must be reported to the Head of School/Function and an accident report form should be completed as well as completing any forms required by the Driver's insurance company. Students on work placement should notify the host organisation and the University Mentor, should they be involved in an accident/dangerous occurence. A TU Dublin accident report form should be completed.
- Those on overseas University business should notify the host organisation, their line manager and the local authority (if required). A TU Dublin accident report form should be completed.
- Accidents will be investigated by the Manager/Supervisor in charge of the area in which the accident occured.
- A Safety Representative has the right to inspect immediately where an accident or dangerous occurrence has taken place, or where there is imminent danger or risk to the safety, health and welfare of any person. To facilitate this, Heads of School/Function endeavour to notify their local Safety Representative immediately in such a scenario. In the event of a serious accident/dangerous occurence, the Safety, Health and Welfare Senior Manager will liaise with the Health and Safety Authority (HSA) and Gardai.
- The statutory reporting of accidents and dangerous occurrences to the HSA will be completed by the SHW Administrator.
